Mumps
Reviewed by Dr John Pillinger, GP
What is mumps?
Mumps is caused by a paramyxovirus. Of the more common childhood illnesses, it is the disease with the longest incubation period and can take as long as three weeks from infection to outbreak, which can make it difficult to track down the source of infection.
The virus is contagious for about two to three days before and up to seven to eight days after the disease breaks out.
How do you catch mumps?
Mumps is a viral infection spread by airborne droplets from the nose or throat. However, it is the least contagious of the five major children's diseases and requires close contact before infection can occur. Though small children can get mumps, the disease is most common after the age of two.
Today, most children are vaccinated against mumps with the MMR (measles, mumps, rubella) vaccination.
The virus enters the body through the airways, then passes around the body in the bloodstream. It can end up almost everywhere - the kidneys, thyroid gland, pancreas, sexual glands and, not least, the salivary gland. The virus thrives in the parotid salivary glands, which lie in the cheeks just in front of the ears.
What are the symptoms of mumps?
Mumps usually begins with two days of discomfort and an increasing temperature. This is followed by an uncomfortable feeling in the jawbone, later accompanied by a swelling of the parotid glands. Often the swelling occurs unevenly, on one side of the face before the other. The body temperature may rise to 40oC and the swelling can feel oppressive and sore. The earlobes stick out and the child's face eventually looks very swollen. The child experiences pain when opening their mouth. In mild cases the swelling may only last three to four days, but it can go on for a week or more.
How does the doctor make the diagnosis?
Usually the doctor makes the diagnosis based on the symptoms.
What complications may arise?
Although the swollen parotid glands and high temperature caused by mumps may be unpleasant, the most serious repercussions involve possible infection of other organs.
In 20 to 30 per cent of the cases of adult men with mumps, the disease infects the testicles (orchitis) causing swelling, pain, soreness and a higher temperature. This often occurs about a week after the disease has broken out, and is a serious infection that may cause sterility.
However, among the few sexually mature men who contract mumps, only half get orchitis. Of these patients, only 10 per cent are affected in both testicles, and even then it does not necessarily cause sterility. If sterility occurs, there is still a chance that fertility may return.
Boys who have not reached puberty and are not sexually mature rarely get this kind of inflammation of the testicles.
The risk of women contracting oophoritis – inflammation of the ovaries – is even smaller than inflammation of the testicles in men. However, should this happen, it has no effect on fertility.
Inflammation of the pancreas (pancreatitis) is a recognised but uncommon complication.
A less rare complication of mumps is meningitis, which may appear 3 to 10 days after the onset of mumps. This is an inflammation of the membranes of the brain or spinal cord. Meningitis is a serious disease, but in connection with mumps it is usually mild. However, it still requires close attention and special care of the patient.
The symptoms of mumps-meningitis are:
Apart from staying in bed while the swelling and temperature is at its highest, mumps demands no special attention.
Prior to the development of a vaccine mumps was considered to be a part of growing up for many children. If the disease is contracted later in life, the risk of the unpleasant complications listed above is far greater.
Children can return to childcare institutions or school when they are feeling well and the swelling of the glands has subsided.
Who should be vaccinated?

The symptoms of mumps-meningitis are:
- headache
- aversion to bright light
- possible vomiting
- typically a stiff neck, leaving patients unable to touch their chest with their chin and causing the head to bend slightly backwards.
Apart from staying in bed while the swelling and temperature is at its highest, mumps demands no special attention.

Children can return to childcare institutions or school when they are feeling well and the swelling of the glands has subsided.
Who should be vaccinated?
Today it is recommended that all children get the MMR vaccination, which contains a vaccine against mumps. This vaccine gives the same lifelong immunity and protection against the disease as the disease itself.
The recent rise in the number of mumps cases among young students is thought to be entirely due to these children having missed being vaccinated earlier in their life.
In the UK, the vaccine is offered to all children between 12 and 18 months, with a second booster dose before starting school between the ages of three and five.

Bike Frame Geometry
The geometry of a bicycle frame is determined by the lengths of the various tubes (seat tube, top tube, head tube etc.) and the angles at which they are joined.
According to Paul Hillbrick, the critical aspects of frame geometry in relation to the fit of a bike are the seat tube angle, the head tube angle, the top tube length and the standover height (which will depend on the seat tube length, whether the top tube is horizontal or sloping and the height of the bottom bracket.)
Look up the geometry of a bike on a manufacturer's website and you should find measurements for each of these aspects. Often others will be included as well.

How to Determine the Correct Frame Size for a Bike
Step1: Measure your inseam. This is best done barefoot. Stand against the wall and put a book between you legs so it pressed right up against your pelvic bone. Make a mark with a pencil on the wall along the top of the book. Measure the distance from the floor to the mark in centimeters.
Step2: Use your inseam measurement to get a rough idea of your road bike size. Multiply your inseam by .65. This will give you a good estimate of you road bike size for bikes measured center to center. Thus if your inseam is 86 cm, you will fit a 56cm road bike (86 x .65 = 55.9). Note that many road bikes are measured center to top. To determine how to fit these bikes, multiply your inseam by .67.
Step3: Subtract an additional 10cm and convert this measurement to inches to get your mountain bike frame size, roughly. Thus, if you fit a 56cm road bike (c-c), you will fit a 46cm - or an 18" - mountain bike.
Step4: Consider top tube length. In many ways, this is the most important aspect of sizing a bike. Two 18" mountain bikes may have different length top tubes. Or a 54cm and an 56cm road bike may have the same length top tube. Given the same top tube length, the bigger bike may be more comfortable in that it will allow you to get the bars up a bit higher.
Step5: Know that women have longer legs and shorter top tubes than men. Unfortunately, most bikes are designed for men, and women often have a hard time finding a bike that will fit them well. Luckily, some manufacturers have begun to make women-specific models.
Step6:Test ride some bikes once you have gotten a rough idea of what will fit you. This will help you determine what is most comfortable for you.

My Brand New Nike Sweet Classic AP (White/Cherry Red)
This is my 2nd Nike in 2 months since the one that I bought from HK in Oct...
Anyway, like the classic look & old school with a little bit style of world well known Converse classic. Nike even add in Sweet to it, hope it would not draw the attention of ants.
The feeling of wearing it flashed me back to primary school time when we were wearing Pallas school shoe that comes with a string of blue line cutting across the shoe base and having the flat sole. Instead of canvas, this comes in leather...
Need some times to break in...this one is for CNY09 (have to find a good reason for this purchase). This was poisoned solely by L....(Damn it...I must control myself better next time.) while he is sourcing for his new working shoe, a Nike AirForce-1 which I think as the name implied would help him getting from his desk to "SOS" desk in split second. And maybe its comes equiped with guided missile and fly him away from batam even before the missile hit "SOS"'s little golf balls.
Here is my Sweet Classic AP White with just enough cherry red as icing on the cake....
sweet baby sweet....yeah....
and some real life pic to goes by here...

Pinguecula:
Pinguecula
Overview
A pinguecula (pin gwe' cue la) is a benign, yellowish growth that forms on the conjunctiva. They usually grow near the cornea on the nasal side. Pingueculae (plural form of pinguecula) are thought to be caused by ultraviolet light and are most common among people who spend a great deal of time outdoors.
This growth does not affect vision, but may cause irritation if it becomes elevated. In rare cases, the pinguecula may gradually extend over the cornea, forming a pterygium.
Signs and Symptoms
Pingueculae are harmless growths and rarely cause symptoms.
Yellowish, raised area on the conjunctiva
Irritation and scratchiness
Dry eye
Occasional inflammation of the conjunctiva
Redness if the area becomes irritated
Detection and Diagnosis
Pingueculae can often be seen with the naked eye; however, the doctor diagnoses the growth with a careful examination with a slit lamp microscope.
Treatment
Because of their benign nature, pingueculae rarely require treatment. Occasionally, the growth may become inflamed, causing irritation and dryness. The doctor may prescribe artificial tears for lubrication and mild anti-inflammatory medication to reduce swelling.

1. Fenix L0D uses 1xAAA battery. And it has the diameter of AA battery but a little longer. Has 5 outputs type: 25 Lumens (3.5hrs) -> 9 Lumens (8.5hrs) -> 75 Lumens (1hrs) -> Strobe -> SOS.
This light is insanely bright for its size, it beat Maglite hands down, even the 3D batteries Maglit not its rival, of course except beating up peoples head.
2. Ultrafire C2 uses 2xCR123 batterry also has 5 Mode Clicky Switch: Mid > Low > High > Fast Strobe > Slow Strobe
This one was bought from DealExtreme, do not have details infor about the lumen output but it is definitely brighter than L0D in every setting, but its not an fair comparison as this using 2 CR123 batt.
see the comparison pics below...

HK 3/4days 2008
Just came back from HK on 1st Oct 2008 midnight, what a tired and buy days in HK.
Was monitoring the weather there last few days before departure, and was quite a worry that Typhoon Jangmi would strike HK, but fortunately for me that its divert its path to Taiwan (pity those residents there).
Took the 8.30 flight to HK and reach there ard 12noon, took their airport express train to Kowloon MTR and board their free shuttle bus to Hotel Prudential. This hotel is located along Nathan Road, the hotel room is well organised with all the necessaries in place, clean, tidy and big enuff for 2 or 3 adults. Their double seem to be larger than the king size in my house???
Very satified with their mattress, very comfortable, soft but supportive. Highly recommended...
Rest and head to Nathan street for window shopping, the street to my opinion is not as clean as this tiny city here but they their sales promoters are definitely better if not good, they tend to give their opinion or suggestion on what fit you...which are most welcome.
First day finish with visiting the Symphony of Lights at TST Avenue of Stars.
2nd day was at HK Disneyland the whole day, nothing much to say about this except one of their attraction which scare my kid till cry I almost cry too, it was Space Mountain...heck its too scary for kid.
The 3rd day was going to Lantau Island to see the Giant Buddha. The scence from the cable was magnificent. Then we head to city and going to the Peak to see HK night view from The Peak.
Last day was spending by shopping ard Ladies market and Sport wear stree. Bought a Nike Air Zoom LX 3+ AP Trainer for myself and my dad got himself a Timberland DL24 Trainer Low. Two of them cost only HK$618, a very good price.... nike cost HK339 (SGD63) and Timberland cost HK279 (SGD51).
